Certification

Under penalties of perjury, I certify that:

  1. The number shown on this form is my correct taxpayer identification number (or I am waiting for a number to be issued to me); and
  2. I am not subject to backup withholding because: (a) I am exempt from backup withholding, or (b) I have not been notified by the Internal Revenue Service (IRS) that I am subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified me that I am no longer subject to backup withholding; and

Certification instructions. 2022 Christmas Boutique Waiver of Liability

The undersigned (referred to hereinafter as the “Vendor”) intends to participate in the Calvary Christian School Christmas Boutique (referred to hereinafter as the “Boutique”) on November 4, 2022, at 701 Palisades Drive, Pacific Palisades, California. Vendor acknowledges that routine school security will constitute the security of the event and no additional security or safety services will be provided to preserve or protect Vendor’s merchandise during the Boutique’s setup, the event itself, or break down.

Vendor agrees to hold harmless Calvary Church of Pacific Palisades, Calvary Christian School, it’s staff, students, and their parents, Calvary Christian School Parents Association, and the Boutique (collectively, the “Boutique Beneficiaries”) from any and all harm, loss, theft, damage to or destruction of Vendor’s merchandise arising out of Vendor’s participation in the Christmas Boutique.

Further, Vendor agrees to waive any legal right or claim, including the right to institute legal proceedings against the Boutique Beneficiaries as the result of any harm, loss, theft, damage to or destruction of Vendor’s merchandise arising out of Vendor’s participation in the Boutique. The Boutique Beneficiaries make no warranty or guarantee, express or implied regarding the safety or security of the area housing the Christmas Boutique, nor the Vendor’s merchandise stored or displayed therein.

ACCEPTED AND AGREED TO:

Vendor Central Cashiering Agreement

ACCEPTED AND AGREED TO:

Thank you for participating in Calvary Christian School’s 2022 Christmas Boutique (Boutique). The Boutique is held by the Calvary Christian School’s Parent Association as a fundraising and community-building event for Calvary Christian School. Funds benefit Calvary Christian School, a non-profit organization.All participating vendors are required to familiarize themselves with and agree to the collection and payment policy as stated below:The Boutique uses a central cashiering system for all purchases. The Boutique collects all payments from customers on the Vendor’s behalf, acting as the collection agent for the Vendor on the event day. Customer payments will be accepted as cash, check, or credit card. Sales slip/receipt books will be provided to each Vendor by the Boutique. The amount collected from the customer will be based on a sales slip prepared by the Vendor.The sales slip will clearly state:

1) Vendor Name and Vendor Table Number

2) An Itemization of merchandise sold

3) A total amount for merchandise sold

4) The Los Angeles sales tax amount on merchandise sold

5) The cash sum of total merchandise sold plus sales tax

6) Shipping charge (if applicable)

The vendor will write up the order on the sales triplicate slip form in the receipt books provided by the Boutique. The vendor will give two copies of the sales slip (yellow and white) to the customer to bring with them to the cashier. The Boutique cashier collects payment from the customer. The Boutique cashier will mark the yellow sales slip as paid. The customer returns the stamped paid sales slip to the vendor in order to pick up the merchandise. The vendor keeps the yellow paid stamp and gives the pink sales slip to the customer. (The Boutique cashier will retain the white sales slip).The Boutique receipt system allows customers to purchase items from multiple vendors with one payment and allows vendors to spend more time with customers selling merchandise, increasing sales potential.

Payment to Vendor:

The Boutique pays participating Vendors the total sales amount of merchandise sold, including the sales tax and any shipping charges, less the Boutique fee of 20% of sales (excluding shipping and tax). The Boutique does not take a % of the sales tax or shipping fees.Payments to the Vendors will be made by check and sent via US mail within two weeks of the Boutique. Please allow 10-12 business days to receive payment.

PLEASE NOTE THE CCS BOUTIQUE DOES NOT REPORT TAXES. IT IS THE RESPONSIBILITY OF THE VENDOR TO REPORT AND PAY ALL STATE, FEDERAL, CITY AND/OR LOCAL GOVERNMENT AUTHORITY FOR ANY TAXES THAT MAY BE DUE AS A RESULT OF THE SALE OF ANY OF THE VENDOR’S PRODUCTS OR SERVICES AT THE CCS CHRISTMAS BOUTIQUE.

I have read, understand, and agree to the above collection and payment procedures of the Calvary Christian School Boutique.

Dear Vendor,

Re: Christmas Boutique Application

Thank you for your interest in Calvary Christian School’s Christmas Boutique! The 31st annual Calvary Christmas Boutique will be held on Friday, November 4, 2022, from 10am-5pm, on our beautiful campus.

The Calvary Boutique Committee invites you to submit an application to participate in this year’s Boutique. Due to size restrictions, an application does not guarantee Boutique participation. You will be notified of enrollment by email and upon receipt of the Vendor Confirmation Checklist. The Boutique Committee anticipates hosting approximately 65 vendors this year. Please turn in your application with necessary documents (as listed in the application) as soon as possible.

If the Boutique is unable to accommodate you this year, your registration fee will be refunded by mail.

Please submit the registration fee with your application to be considered for the Christmas Boutique.

The registration fee constitutes:

* $120 dollars per table, (no less than 6 ft).

* $50 per “rack space”. This is for space only. You will need to provide your own rack. (5 1/2’ by 2’ to be used ONLY for rolling racks forclothing or coat type racks for handbag/hat display)

Each vendor will be assigned a table location. Vendor table locations are carefully planned prior to the event to ensure maximum success for all vendors. Unfortunately, we are unable to accommodate last-minute relocations. The Boutique Committee will do its best to take in all considerations, but cannot guarantee special requests. Due to location limitations, the Boutique is unable to accommodate vendors bringing their own tables or wall type displays.

Vendors will be required to pay 20% of total sales, exclusive of sales tax to Calvary Christian School.

The Boutique Committee works to maximize successful participation by assembling a wide variety of unique merchandise and price points. Participating vendors are based on the type of items offered as well as past performance at the Boutique.

Please fill out all types and ranges of merchandise you are planning to sell on the application, to assist in reducing the number of similar products and categories offered at the Boutique. In the event a handful of Vendors specify similar-type products, we may request alternative product lines. Shop owners may choose to consider selling specialty items not normally carried in their stores. Returning vendors, please consider offering new and additional items from the past year.
